Required items on Application

Dear Applicant:

When applying to our Urology Residency Program, the following items must be submitted in order to have a complete application:

1)  Must have at least 2 year’s training in a United States medical school in order to be considered for review. 

2)  Application Form (on the ERAS program)

3)  Three Letters of  Recommendation

4)  Dean’s Letter (When Available must be original)
