Disaster Recovery Planning Forms and Information

Your unit’s emergency preparedness/disaster recovery plan is due by August 31.

Your unit’s Emergency Contacts Form was due by May 21.

Information:

In today’s environment, having a current emergency preparedness/disaster recovery plan is crucial. It will help you anticipate and prevent or mitigate problems, and it will help you restore critical business functions more quickly in the event that there is an interruption in services.   The University requires each unit in the School of Medicine to have a plan that outlines how it will handle emergencies. We have prepared instructions and sample forms to help you develop your plan:

Instructions: 

You should start with the document titled, “A Unit Guide to Disaster Recovery Planning.”

Marge Sideb ottom will make a presentation on emergency preparedness and crisis management at the May 7 HSF/SOM joint administrators meeting. Her discussion should help you understand the issues and the sorts of items that your plan should address. 

We ask that each unit supply the names and contact information of primary and secondary contacts.  These individuals will be the contacts for the Health System’s Command Center Team.  In case of an emergency, they will be sent a message in order of the contact method that is most likely to reach them.  See the example on the attached Emergency Contacts Form, and use this form to supply the information on your primary and secondary contacts.
